Formal Use

     Buenos días, señor. ¿Cuál es su nombre?
     Mi nombre es Roberto Martinez.

Informal Use

     Hola! ¿Cuál es tu nombre?
     Mi nombre es Ricardo Vera.

To give all three:

     ¿Cuál es tu nombre? - Mi nombre es [...].
     ¿Cuál es tu apellido? - Mi apellido es [...].
     ¿Cuál es tu apodo? - Mi apodo es [...].

Cultural Awareness

"¿Cuál es su/tu nombre?" and "Mi nombre es..." are usually heard more in Latin American countries than in Spain.
